Does it come with a filter and straw?

There is no straw as you drink directly from the rubber top on the lid of the bottle. There is a cover that screws over the rubber you drink from for transporting and keeping it clean.

Does it filter fluoride?

LifeStraw products do not filter out fluoride.

Does the cap fit a nalgene? Can I buy only the cap & filter and use it on my Nalgene or do I have to buy, yet another, bottle?

No.. roughly 3 inch diameter cap.

Is it supposed to spew water due to design?

All I know is that mine spewed water all over me every.single.time I opened it on the plane (1st, 2nd, 3rd, 4th time etc) no matter what trick I tried. They say to blow back into the straw to prevent this but that doesn’t work. It even spewed all over me when traveling over hills! Defeated the whole purpose of why I got it (for travel so I didn’t have to buy bottled water at the airport). But also not usable for even just regular, daily use due to the poor design. I hated it so much I left this expensive bottle at the last hotel I stayed at while traveling as I saw no reason to bring it home.
