It does twist. The handle should twist will extend to 52”. Maybe it’s stuck. At the bottom of pole near brush there is a black piece on pole that is where it should pull out from. May twist that part could be stuck.

There is a YouTube video for this old house. It shows a mixture you can use. Search for grout cleaner.

What diameter hole will the replacement heads fit?

I love this brush. I used it on my tile grout and it works perfectly. I just dip it bucket of water with a little bleach and brush my grout. I would highly recommend this product.

How to stop brush head from pivoting?

I haven't had a problem with it pivoting---though you do have to tighten the screw-on head fairly snug so it doesn't unscrew and thus pivot Hope this is helpful.
