You can build a mini water dam around the hole with plumbers putty, spray water on the drill bit, or use running water to cool it.

It will drill concrete but may not drill deep enough.

Will this work with hard backer board?

I haven't tried it on backer board. I may but it would be slow going. I normally use a standard hole saw on hardi backer.

Did anyone use this to cut porcelain tile?

I have. Stone, ceramic and glass as well. Drilax makes good bits. Just be sure to keep the surface flooded with water or they won't last.

Do you need a special drill attachment to use this on a Makita drill?

No special attachment is needed. It fits in a typical drill chuck. The hole saw will need a drill with enough power to perform best. The hole saw needs to be contained so that it does not run across the tile you are cutting. I created a square corral that fits the hole saw snuggly and then clamped the corral to the tile. Cut 3 pieces at least 2 inches wide and the length equaling the saw hole diameter. Glue one of the three pieces to two longer and wider pieces flush to the ends of the larger pieces. Then position one of the 2 remaining pieces to the 2 larger pieces using the 3rd piece as a gauge. Overlapping strips can be applied to the top of the corral to reinforce it.
