It can be used in a glass bird bath as long as there is a plug-in available.

Does this move the water enough to keep mosquitoes from breeding in the bath?

Yes, it helps prevent mosquito breeding if there is sufficient water depth for a more vigorous flow.

Does the bird bath need to be level for this to work?

It should work fine in a bird bath as long as the water is deep enough, about an inch and a half minimum.
