It was a very small hole, about the size of a rivet that could go through a button.

What keeps the screw from loosening and having to replace the button multiple times?

These rivets come with a small Phillips screw driver. You make a hole with the screw driver. Push the backing piece through the hole. While holding the backing piece in place with one hand, screw the rivet on the front by hand and hand tighten. Then hold the rivet with one hand and tighten the backing piece with the screw driver. Turn the backing piece until it is tight and will not turn anymore. At that point, it's tight enough to stay in place unless you decide to loosen and remove the backing piece with the screw driver. It's amazingly easy to replace a rivet or add a new one to loosen or tighten the jeans. I did the job for the first time in five minutes.
