How does this attach to floor so it does not scoot off of step when going up or down?

You can use screws but it is designed to be stable when used as portable, has a 1 inch lip to put on landing at high end. The longer the ramp, the less steep the incline will be and the more comfortable for the passenger.

What does the assembly involve?

It’s been quite a while since I purchased this. Seems like I remember assembly to be very easy.

Would this be appropriate for loading a mobility scooter into the back of a van? Loading manually, not riding it.

I think so. You would be able to "drive" or roll it up the ramp and into the vehicle.

What is the inner width of this ramp (between the 2 side walls)?

It’s about a quarter of an inch. I took the hinges out and bolted them together.
