It means nothing, they are making it look like UMF rating which is the certifying agency for real authentic Manuka honey potency (higher rating means more health benefits). This honey is not certified by UMF so it seems sketchy to have 5+, 10+, etc... on the label. Look up legit brands such as Comvita to see the true UMF certified logo and ratings.

Should the honey be thick (like chrystalized) when opened first time?

In my experience it always is. It's that thick all the time. I add it to my herbal infusions.
