What diameter hole is required?

There isn't a pilot hole required. You just buy the size of hole saw you need and slowly start drilling with just one edge (holding your drill on an angle) until it starts to cut a little groove in the tile or whatever you're cutting. As the groove forms you slowly bring the drill straight until the hole saw is grinding away on the entire surface. Sounds a little tricky but it's no problem. Keep a wet sponge or wash cloth beside the bit as you're using it and keep the surface nice and wet.
