If it says fgfs101zw3 it is not zwave plus. Yes mine all say fgfs101zw5 on the box.

Do you need to have a hub for this to work to get alerts?

Flood Sensor will inform you in case of flooding by sound and flashing led. But if you want to get notification/alarm you need to have a controller - Home Center.

Can you have multiple external sensors connected to it? Like one under the dishwasher and one under the sink?

It has contacts internally that you can connect an external sensor and STILL have the device itself work as a sensor. So in your case you would place the device under the sink and run another sensor from it to under the dishwasher. If either location detects water the sensor will activate.

The Fibaro FGFS-101 ZW5 FGFS101ZW5 Flood Sensor is a highly rated Z-Wave Plus certified device. It can detect water leaks and give sound and flashing LED alerts, or, with a controller such as Home Center, send notification or alarm alerts. The device has contacts that allow an external sensor to be wired in as well. It is versatile, and customers have used it in various configurations to monitor different locations such as refrigerators, sump pumps, AC and dehumidifiers. The battery life can last up to a year and has worked Consistently for most users. However, a few users have reported issues with inConsistent notifications, and the battery discharging rapidly when an external power source was added. A couple of users also experienced less than satisfactory support from Hubitat and found the driver to be incomplete. In general, the Fibaro FGFS-101 ZW5 Flood Sensor is a solid, reliable choice for monitoring water leaks with Z-Wave Plus technology.
